Investigation of Unexplained Deaths in Basankusu

On March 17, 2025, the ACODD team conducted a comprehensive investigation in Basankusu Health Zone, specifically in Ekoto Health Area, Bomate Groupement, covering Lokombo and Ekalankoy villages. Using a standardized questionnaire, the investigation identified a critical time frame of less than 48 hours between symptom onset—characterized by headaches, chills, generalized muscle stiffness, uncoordinated movements, and severe fatigue—and death. Alarmingly, verbal autopsy reports indicated rapid post-mortem decomposition within hours.

Environmental assessments revealed that surface water wells, undrained since January, are located downstream of extensive oil palm plantations and surrounded by three-month-old seedlings. Community members, though hesitant to disclose details, acknowledged the clandestine use of herbicides and industrial fertilizers. This ecological and public health context requires an urgent response to assess potential chemical and biological risks.

Recommendations

– Conduct urgent toxicological analyses of water sources.

– Strengthen epidemiological surveillance and establish structured community engagement to overcome sociocultural barriers to information.

– Construct improved water sources to ensure safe drinking water for the population.

– Provide psychological and humanitarian support to the affected communities, particularly in Bomate.

– Continue active case finding and deepen investigations along the Waka axis, where 28 deaths have been reported.
